Detailed Description
Patients take a 10-minute health quiz with questions covering
lifestyle risk, health history, education, and patient care satisfaction.
Sections include daily activities, feelings, social activities
and support, pain, physical fitness, health habits.
A summary and a three-page feedback report are provided for each
patient, and are designed for the physician to share when discussing
health risk or problems, health education, and disease management.

When to administer this assessment
No protocol exists for the administration of this instrument.
Can be used as a baseline assessment for new patients or at physician's
determination of need and re-administered at set intervals to
monitor changes and improvements in patient health, lifestyle
choices, or care satisfaction.

John Wasson, MD., is the Herman O. West Professor of Geriatrics
at Dartmouth Medical School and a Distinguished Professor of Medicine
and Community and Family Medicine. The range of Dr. Wasson's research,
articles, videodisk products, and books is broad; it covers subjects
ranging from pediatrics to geriatrics. He serves on many national
editorial boards and advisory panels. His first book, the award-winning
Common Symptom Guide has been in continuous print since 1975.
For more than twenty years, a cooperative network of physicians,
nurses, and researchers affiliated with Dartmouth Medical School
