Launch Forms and Update Forms
Patient Tools makes our Launch and Update forms simple to use
yet powerful in their ability to collect pertinent patient data
that is essential for accurate measurements and reports.
Using a pre-treatment Launch form, you can collect information
not collected by the assessments, display information collected
during a previous visit, or pull information from an existing
Electronic Medical Record (EMR). You can even input the course
of treatment for a patient with a pre-existing condition.

You can change the selection at any time, as necessary, and the
system logs the date of change to let you evaluate responses from
before and after.
The post-treatment Update Form allows you to capture clinical
lab test results or course of treatment prescribed during the
patient's office visit.

You can assign a diagnosis for a new patient or a new condition,
enter medication prescriptions, or log the results of laboratory
tests.
The critical volume of data your practice needs to track and
evaluate is collected on these configurable forms and is centrally
stored, allowing you to easily pull the data in real time to create
aggregate, dashboard reporting for analysis.
Data Import/Export Capability
A unique benefit of using an INFOSuite is the ability to "push"
and "pull" data to and from other electronic records
in your enterprise. Pull information from electronic files into
your local, Patient Tools database, centralizing demographic,
insurance, provider, and financial information and expanding the
range of variables you can analyze.
Or, if you've implemented a complete Care Management System,
export the data collected by the PatientINFO system into your
own system and incorporate the information you need into your
reporting functions for comprehensive analysis.
Combining the strength of the data from these systems gives you
the high level of quantifiable documentation you need to make
informed treatment decisions and analyze system efficiencies across
your entire practice.
