Patient Tools offers a range of automated INFOCore tools. Each INFOCore is condition-specific, with demographic and health history information, adherence evaluations, and patient current health concern scales.
Each INFOCore is based on a basic intake (inventory) question list and an additional set of disease-specific questions. Sections are designed to collect basic demographic and health history information and help evaluate adherence issues, patient quality of life, and pre-screen for comorbidity on an on-going basis.
How does it work?
An INFOCore is like the typical patient interview in the practice office, with a couple of significant advantages.
A typical INFOCore contains two types of question sections – the "baseline" health and demographic data that collected only during the first administration of the tool, and "visit-specific" questions that are repeated with each patient visit or according to other, predetermined criteria.
Instead of spending the office visit time gathering background and history information, the physician receives a printed report with all of the relevant data before they enter the examination room. The time usually spent gathering this basic data can be spent focusing on the patient's needs.
This same printed report gives the results of the visit-specific questions. These help monitor potential adherence problems, pre-screen for the existence (or development) of comorbid conditions such as depression, and list the patient's probable immediate problems or health concerns.
The information is saved in a database that allows for trended reports, to show changes in individual patient condition over time. Re-administering the INFOCore before or during subsequent office visits gives the physician critical information on changes in the patient's condition or symptoms.
